Our first ever Centre for Population Health Programme supporting 31 leaders across NWL and NCL Integrated Care Boards and partner organisations aiming to improve population health and equity for 1.3 million people in 13 London Boroughs.

We start the journey exploring data sets and analysis methods for PHM leadership, and learn ways to couple this with co-production methods to gather insights and practical plans for improving population health and equity in practice.

We explore the concept of health equity and methods for tackling health inequalities in ways that can really make a difference as PHM leaders.

We consider how best to partner with communities, community organisations and other key partners to get this right in practice as PHM leaders.

We help to consider what it takes to be a system leader for PHM and how to work across boundaries effectively to drive improvements for population health and equity.

We consider what it takes to create lasting positive change, using measurement and evaluation methods to enable further progress along the way.
